Even tho I was slow getting my paperwork filled òut because I'm not tech savvy, the Dr called & did our visit right as I finished & did a quick and courteous appointment. My papers were sent shortly after along with links to my state board & everything I needed. Thanks Elevate, for an awesome experience!

Update Sept 8th: After speaking to a representative and thoroughly explaining to her I do NOT have a condition the doctor or nurse practitioner as a diagnosis to get the medical card approved for me. It is very illegal for a licensed professional to grab any diagnosis they feel just to get a medical card approved. The diagnosis they gave me was never a word that came out of my mouth on a recorded line. I never typed that word in any of my my patient intake forms or patient history. Now, if on my intake forms and patient history I listed multiple diagnosis that would have a medical card be approved for me then I do find it appropriate to pick from a list that is within the patient list and documentation. It is not appropriate to pull a diagnosis out of thin air and legally document that I have something when I don't. I am very disheartened that after I thought I had cleared this up that two certifications was sent to the state and one of them being the very thing I am not diagnosed with. The very thing I thought I had resolved and its not.
So, its very imperative patients to pay attention to your paperwork.

8/6 I did my appointment on a Sunday and I need assistance and I am guessing that is why I can't get through. I was going to finish my application with the state but I am unable to do so. For one, there is a doctor listed on the form that I did NOT see. I have the qualifying conditions to receive a medical card so why falsify my record by listing anorexia when that is not something I am diagnosed with or was listed on any of the forms I completed for this company. More importantly, I can't apply online without the physican registration number and address so why has that been left off of my certification paperwork?

Lots of issues trying to do anything on the site. The chat bot kept coming up and blocking any thing I needed to do. It took me over 2 hours to get my renewal placed. I had to call the Dr. To finish the renewal process. Dr Miller was the highlight and I give him 5 stars. The rest of the process might have been better if the chat bot didnt automatically pop up within seconds of trying to do anything.

Effortless and I appreciate the discount. And I might add after a minor snag in my recertification, attentive and efficient.
